Migraines are thought of as normal neurological functions interspersed with episodes of headaches.  At least 20% of migraineurs meeting the International Headache Society criteria have changed the pattern from episodes of migraine into patterns of chronic headache.

 

Chronic headache often does not return to normal neurological function.  This evolving process from episodic headache to chronic headache is called transformation.  In the population segment of chronic daily headache, migraine appears to be a progressive neurological disease.  The decline of normal neurological function between episodes of migraine greatly exacerbates the migraine syndrome.

 

Transformation is a process that occurs usually over years.  Usually the migraineur starts off having acute episodic migraines.  As someone continues to have migraines over time, the pain can occur more often and start merging together with a mixed headache pattern of lower severity migraines intermingled with more severe episodes.

 

The mixed headache pattern can further transform into chronic daily headache.  These transformed headaches can be caused by rebound phenomenon or can happen by reasons that are not fully understood at the present time.

 

A threshold can refer to a horizontal piece of building material beneath a doorway or window, but the meaning is different in medical jargon.  Threshold refers to the point at which stimulus or stimuli are of sufficient intensity to produce an effect.  The threshold for migraines can be lowered (migraines may happen more easily) by having repeated migraines.  Facilitation or kindling describe this phenomenon.

 

Migraines make the sensitive body more sensitive.  In other words, migraines breed migraines.  The lower threshold for migraines is made easier as though kindling were put on a fire.  Uncontrolled migraines not only cause more migraines but eventually cause transformed migraines.  A severe stimulus, injury, illness, or stress can provoke the nervous system to generate uncontrolled migraines.  The migraine pain changes over time.  This change is called transformation.

 

You can imagine one of your kid’s transformers changing from a robot to a plane or car, but can you imagine your migraines changing from a more classic presentation to chronic neck pain, sinus pain, or constant headache.  Well believe it or not, over time migraines do transform.  Usually a decade or more is necessary for transformation of episodic to chronic migraine.

Studies presented at the 2019 ACR/ARP Annual Meeting in Atlanta revealed some worrying facts.

 

Opiates have become commonplace. Seen as a quick temporary pain relief fix, they appear to be handed out like candy. Not only are patients becoming addicted, opiates also come with a host of other problems, some of which can be very dangerous.

 

The first study showed a big increase in opioid-use disorder hospitalizations among people with five musculoskeletal conditions including osteoarthritis, rheumatoid arthritis, gout, fibromyalgia, and lower back pain.

 

Because all these conditions involve chronic pain, sufferers are at extreme risk of receiving large opioid prescriptions.

 

People who constantly take large amounts of opiates will inevitably become dependent on them, not to mention, their effects will eventually wear off. Weaning yourself off opiates can be a long and arduous task, and in some cases, sufferers simply can’t come off the drugs due to addiction, and in other cases, the withdrawal symptoms upon cessation of use are devastating. This is the definition of opioid use disorder.

 

Researchers at the University of Alabama at Birmingham were not interested in the occasional accidental overdoses that happen, but rather in the use disorder.

 

They found hospitalizations increased 3.5 times for lower back pain and 24 times for gout with the two types of arthritis in-between. They also discovered arthritis patients who use opioids actually get very poor value for their money, especially when considering the addiction risk.

 

They collected 23 previously published randomized controlled trials with 11,402 subjects. All the subjects had arthritis.

 

At two, four, eight, and 12 weeks, the subjects reported a very small improvement in pain and function, but it was indeed very small.

 

In addition, they reported no improvement in quality of life, meaning that the opioids had not improved their quality of sleep, their depression levels, and their willingness to participate in normal life activities.

In the past two years, several scientific articles appeared in academic journals that discussed the effectiveness of a new, non-invasive and hugely effective treatment for the relief of arthritis pain, inflammation and the restoration of movement in affected joints .

While 9 of the conventional therapy patients needed a knee replacement after 6 years, only 1 patient in the combined therapy group did, showing that low-level laser therapy is extremely effective and should be incorporated in arthritis treatments.

 

Low-level lasers are simple, non-destructive, red or infrared light between the wavelengths of 600nm and 1000nm (nanometres).

 

They’re very safe and you can even get similar home-use devices.

 

The exact mechanisms through which lasers relieve arthritis symptoms are not yet understood, but it is known that light at these wavelengths enters cells and warms them. This seems to promote energy production inside the cells together with other poorly understood chemical changes.

A new study out of Great Britain and released by Cambridge University is the latest in a long list of studies proving the dangers of blood pressure drugs.

 

It reveals the alarming connection between a common class of blood pressure medications and kidney failure – causing death in a big percentage of those who suffer from it.

 

A class of blood pressure medication known as ACE inhibitors, used widely all over the world, was studied in the landmark research.

Two new Canadian studies presented at The American Heart Association’s Council on Hypertension Scientific Sessions, revealed some weird results.

 

Your high blood pressure may actually be direct results of how you were born.

 

And it’s probably in a totally different way than you would think.

 

According to the first study, people born at or before 29 weeks of pregnancy have smaller kidneys relative to their body size than those who were born at full term.

 

They also tend to have slightly higher blood pressure when they are already in their 20s.

 

The second study also compared pre-term adults in their 20s with full-term adults at the same age, finding that the cells in their blood vessels were slower to form colonies and, as a result, that their bodies’ ability to form tiny blood vessels (called capillaries) was impaired.

 

Capillaries are the blood vessels that carry oxygen and nutrients to most of the cells around your body.

 

Moreover, the pre-term adults had higher systolic blood pressure and an enlarged left ventricle of the heart.

 

Both these research teams warn that we should not interpret their results as a suggestion that all adults who were born pre-term will suffer from high blood pressure and heart disease. Their results show only a small increase in blood pressure by the time these people get to their 20s, 5 mmHg to be precise.

 

While 5 mmHg might not be enough to worry about, the chief researcher in both studies says that  people's blood pressure increases as they age , and that slightly high blood pressure during our 20s may turn into seriously elevated blood pressure in our 40s, 50s, and 60s.

When a child regularly feels down and does not exhibit an interest in playing or going to school, it may already be signs of Chronic Fatigue Syndrome (CFS). This is a complex disorder characterized by profound fatigue and is not improved by bedrest. It may even be worsened by strenuous physical activities or excessive mental exertion. 

 

Children with CFS usually functioned at a much lower level of strength even before the onset of the illness. A child suffering from CFS may also show signs of general weakness, muscle pain, impaired memory or poor mental concentration, insomnia, and post-exertion fatigue that lasts for more than 24 hours.

 

To better understand this mysterious illness that afflicts thousands if not millions of children around the world, a team of researchers conducted a study to determine the connection between this specific illness to stress and emotional abuse. According to the U.S. Centers for Disease Control (CDC), childhood trauma increases the child or adults' risk of Chronic Fatigue Syndrome by three to eightfold.

 

A separate study based on the data from the Swedish Twin Registry shows that stress is a triggering factor for the said fatigue syndrome. It also shows that emotional instability is a significant CFS risk factor, although genetic and family factors also determine whether this personality trait leads to excessive and regular fatigue.

 

Researchers from CDC support the hypothesis that CFS represents a disorder of adaptation that is promoted by early environmental factors such as lack of nutrition and emotional support --- which, later on, leads to failure to adequately respond to physical, mental, and emotional challenges.

 

In their separate study, researchers Karolinska Institute have found that stress is a significant risk factor for chronic fatigue-like illness, the effect of which may be buffered by genetic influences. Emotional instability recorded 25 years earlier among research subjects has been associated with chronic fatigue through genetic mechanisms contributing to both personality style and expression of the disorder.

 

The study was culled from a community-based sample of more than 56,000 residents of Wichita, Kansas. At the time of the study, they had identified 43 people with ongoing CFS. For the study, they compared these people to 60 matched people without fatigue. A key finding was that severe childhood trauma was linked to a higher risk of CFS. There was an eightfold higher risk of CFS with childhood sexual abuse, a 5.9-fold higher risk of CFS with childhood physical neglect, a 4.6-fold higher risk of CFS with childhood emotional neglect, a 4.3-fold higher risk of CFS with childhood physical abuse and a 2.9-fold higher risk of CFS with childhood emotional abuse.

 

These studies also suggest that CFS is part of a spectrum of disorders that are associated with childhood adversity. In adulthood, these disorders frequently manifest or worsen in relation to acute stress or challenge. These disorders might reflect the brain's inability to adapt or compensate in response to challenge, leading toward maladaptive responses and ultimately disease.

 

Other studies made on more than 19,000 twins born in Sweden revealed that emotionally instability was linked to a 72% higher risk of CFS symptoms. Emotional instability is a personality trait and tends to result in low self-esteem and feelings of anxiety, depression, and guilt.

 

In conclusion, emotional instability is a serious risk factor for CFS, whether a person with this personality trait actually gets CFS depends on unknown genetic and family factors. Thus, emotional instability is an indirect risk factor for CFS, while stress is a direct risk factor.

The Centers for Disease Control (CDC) describes Chronic Fatigue Syndrome as a distinct disorder with specific symptoms and physical signs, based on ruling out other possible causes. The number of persons with CFS is unknown.

 

Symptoms of Chronic Fatigue Syndrome may be similar to those of common viral infections such as muscle aches, headache, and fatigue often developing within a few hours or days and lasting for six months or more.

 

Main symptoms:

 

·         Fatigue or tiredness – a kind of exhaustion that has never been experienced before by the patient to this extent before (new onset) which lasts for at least six months and not relieved by bed rest

·         Fatigue that is so severe it restricts activity (serious fatigue develops with less than one-half of the exertion compared to before the illness)

 

Other symptoms:

 

·         Fatigue lasting more than 24 hours after an amount of exercise that would normally be easily tolerated

·         Mild fever (101 degrees F or less)

·         Sore throat

·         Lymph node tenderness in the neck or armpit

·         muscle weakness all over or multiple locations, not explained by any known disorder

·         Muscle aches also known as myalgas

·         Feeling unrefreshed after sleeping an adequate amount of time

·         Headaches, different from previous headaches in quality, severity, or pattern

·         Joint pain, often moving from joint to joint (migratory arthralgias), without joint swelling or redness

·         Forgetfulness or other similar symptoms including difficulty concentrating, confusion, or irritability

 

Once you experience persistent, extreme fatigue, with or without other symptoms of this disorder, contact your professional health care provider. Other more serious disorders can cause similar symptoms and should be ruled out.
